Physical Therapy Appointment Cancellation and No Show Policy

 

It is the policy of our office that you need to inform the office before your appointment time should you needs to cancel or reschedule. If you do not contact our office or you do not show up for your appointment, a charge of $25.00 will be billed to your account.

 

For optimal results and successful outcome you need to comply with your treatment plan as per the recommendation of your Physician/Surgeon/Physical Therapist. We will make every effort to reschedule your appointments should the need arise. 

 

Cancellations: If you cancel 3 times in a month without notifying the office 24 hours in advance, we will be not be able to schedule appointment times in advance and can only be scheduled only on a day-to-day basis as per the availability of appointments.

 

No Shows: If you does not show up for your appointment 3 times within a month, you will be discharged and the referring doctor will be notified.

Massage Therapy Cancellation Policy: All Massage Therapy appointments require a 24hr cancellation notice or else there is a Cancellation with out notice fee of $25 per cancellation or your copoun or gift certificated will be deemed redeemed.

Massage Therapy Resscheuling Policy: All Massage Therapy appointments require a 24hr cancellation notice, if you wish to rescedule your appointment within 24hrs of your appointment there a charge of $15.00 for rescheduled appointment or your copoun or gift certificated will be deemed redeemed.

Massage Therapy No Show Policy: All Massage Therapy appointments require a 24hr cancellation notice, if you No Show to your scheduled appointment there a charge of $30.00 for rescheduled appointment or your copoun or gift certificated will be deemed redeemed.
